AFFIDAVIT OF LOSS
A document which declares the physical loss of a security. It often happens through destruction in a flood/fire or theft. All details regarding the loss is enveloped on the affidavit, including the name of the owner and any information regarding the security such as the date of issuance and serial number. A letter of indemnity, which requests the replacement of the security, may be issued once the statement has been made.
