IRS FORM 4868
A form that taxpayers must submit if they desire to prolong the amount of time they need for filing their tax returns. Provided by the Internal Revenue Service (IRS), taxpayers should supply the following information: with the individual or married couple’s name(s), address, Social Security number(s), estimate of total tax liability for a year, total payments, remaining amount due (if any), and amount being settled (if any).
